Sponsored by the Diabetes Modernisation Initiative, this lecture will describe both the epidemiology of chronic illnesses (type 2 diabetes and heart disease) as well as the results of a randomized trial of the TEAMcare intervention which is a multicondition collaborative care intervention.

This workshop includes some teaching and live demonstration, video presentations, group and pair work, particularly practicing CRT tasks. The aim is for trainees to gain enough working knowledge of CRT and to have had sufficient practice of CRT tasks to be able to begin therapy with clients under supervision.
