An increasing proportion of the population is living with long term health care issues. Government policy has dictated a shift of emphasis to patient self-management and active participation in care which is increasingly delivered outside hospital.
Main Aims
- To understand patient and carer experience of living with long term conditions
- To develop and evaluate interventions which maintain optimum function and improve symptoms and quality of life
