Maudsley Treatment Inventory (MTI)

The Maudsley Treatment Inventory (MTI) is a clinician-rated instrument designed to support the systematic assessment of previous medication treatments for depression. It provides a structured method for recording antidepressant medications and recognised augmentation strategies, alongside information on dose adequacy, duration, tolerability, adherence and clinical response.

The MTI was developed by academics and clinicians in the Centre for Affective Disorders at the Institute of Psychiatry, Psychology & Neuroscience, led by Professor Anthony Cleare.

It is intended to support clinical and research assessment of treatment-resistant depression (TRD). It is most commonly used as the instrument for rating the treatment failure dimension of the Maudsley Staging Method (MSM), a multidimensional approach to staging resistance in depression.

Why use the MTI?

Accurate classification of prior antidepressant treatment is essential when determining whether a depressive episode is truly treatment resistant. Routine clinical records often lack enough detail to judge whether recommended treatments were delivered at an adequate dose and for an adequate duration.

The MTI addresses this by:

  • Providing a comprehensive list of antidepressant medications and augmentation strategies
  • Allowing structured rating of dose adequacy in line with prescribing guidance
  • Supporting consistent and reproducible assessment of previous treatment trials
  • Improving the reliability of TRD staging in both clinical and research settings

Using the MTI

The MTI is typically completed by a clinician or trained researcher using information from clinical records and/or patient interview. It can be used for single or multiple depressive episodes and is intended for use alongside symptom severity and episode duration measures when completing the Maudsley Staging Method.

An update of the MTI based on the most recent edition of the Maudsley Prescribing Guidelines is currently underway. The updated version will be uploaded on this page when it becomes available.
