Raising concerns

The Faculty of Life Sciences & Medicine (FoLSM) is committed to creating a culture that allows all members of our community and the wider public to raise concerns about patient safety, the standard of care and/or of student training in an open and safe way without fear of negative repercussion. Therefore, we have set up an online reporting system for raising concerns.

Our online Raising Concerns forms are designed to help students, educators, members of staff and the public report concerns they may have about:

  • A student (MBBS, MPharm and/or Physiotherapy)
  • A member of Faculty
  • Patient care
  • Education and training

Confidentiality

In line with the GMC, GPhC and CSP guidance on ‘Duty of Candour’, we require you to identify yourself by providing a contact email address when you use the Raising Concerns form. As far as possible, we will respect your confidentiality during any investigation we undertake, but it is not possible to raise a concern anonymously.

How to raise a concern

If you are a KCL MBBS student, before you proceed to the Raising Concerns Form, please ensure you have carefully read the guidance and resources on KEATS here.

If you are a member of the public or a KCL colleague, please see select the appropriate links to report form by programme below;

Once your report has been received, you will receive email confirmation from the FoLSM Student Support Team within 2 working days. Your report will then be assessed by senior staff within the School as appropriate.

Once investigations have been concluded, you will receive confirmation from the FoLSM Student Support Team. It may not always be possible, or appropriate, for the details to be shared with you.

Professionalism

If you are a KCL member of staff and have a concern about a student’s professional conduct, it is likely that it will be more appropriate that this is investigated under the faculty’s professionalism policies. You can submit a Professionalism Report here.

Please note, this system is not designed to support students in crisis – if you are a student in crisis, please access support through one of these channels.
